Map of Medicine Launches Implementation Toolkit

Map of MedicineMap of Medicine has launched a new toolkit dedicated to helping organisations to implement the Map successfully. Based on the Map of Medicine Adoption Framework and the experience of early adopter communities, the toolkit incorporates some of the processes that facilitate a successful implementation. It provides step-by-step hints, tips, templates and guidance on how to manage a Map implementation, including running pathway localisation projects, communicating with users across different care settings and driving behavioural change.

The Map supports service designers in decisions about local services planning through the provision of best practice information. It enables adaptation by local groups, such as GP consortia, wanting to quickly make care pathways relevant to local circumstances. When used as part of a service review or redesign, the patient-focused output forms a comprehensive guide of the local health system across all care settings.

The evidence-based complete care pathways provide the starting point for the development of locally-relevant pathways across health and social care. Local groups wanting to implement the Map themselves can use the information provided in the free implementation toolkit to support their deployment to benefit from improved clinical effectiveness, patient safety and patient experience.
