Map of Medicine Launches Implementation Toolkit

It enables adaptation by local groups, such as GP consortia, wanting to quickly make care pathways relevant to local circumstances. When used as part of a service review or redesign, the patient-focused output forms a comprehensive guide of the local health system across all care settings.

The evidence-based complete care pathways provide the starting point for the development of locally-relevant pathways across health and social care.
